Sport Physicals Are Accepted Online Only

Please follow instructions:

Step 1: VISIT  homecampus website  for PARENTS & STUDENTS. Select CA.

Step 2: Select START CLEARANCE HERE. Type in KASTNER. Confirm school address.

PARTICIPATING in MULITPLE SPORTS? Use Add New Sport button.

Step 3: Enter 2026/27 School year. COMPLETE ALL REQUIRED FIELDS

Student information, Parent/Guardian Info, Medical History, Signature form, CUSD Pre-

Participation Screening form signed or stamped by a doctor, take a picture with cell phone

and UPLOAD to Home Campus. Sierra Vista Children's Health Center is available by

appointment ONLY for sports screening. Fee is $30.00 Cash/Credit. Please call during

school days/hours 559-327-7976.

Located at 510 Barstow Ave, Clovis. Available to children up to 18 years.

Step 4: Click SUBMIT COMPLETED APPLICATION- MUST BE DONE PRIOR TO TRYOUT

Step 5: Keep an eye out for an email with CLEARANCE information for your student
